Patient referral process
Patients have access to the Colorectal Program the following ways:

• You can recommend a patient directly to the program by calling 398-2489 (Maryview) or 889-4231 (DePaul)
• A patient’s physician can refer them directly to the offices:

• Once it has been determined that your patient will be admitted into the program, a care coordinator will contact the patient to complete a program intake form. The patient care coordinator will also contact your office for all pertinent records and test results.

It is our goal to ensure a streamlined process with minimal interruption to your office staff.
